Application Checklist
Withholding pertinent information requested during the application process or giving false information will make students ineligible for admission, subject to cancellation of admission if admission has already been granted, or subject to dismissal if already enrolled. Applicants will need to certify all statements submitted during the application process are correct and complete.
Students applying to a degree (MBA or MS) program should submit the following:
- Kelley Direct application – Part I
- Kelley Direct application – Part II (including application fee)
- Official academic records from all post-secondary institutions attended
- English translation (literal) of official academic records if not in English
- Résumé
- Personal Statement (A statement, approximately 500 words, that identifies your academic goals, career objectives, why you are applying to this program, and the qualifications you have that make you a strong candidate for this program.)
- One letter of recommendation
- GMAT score report
- TOEFL score report (if native language is not English AND if you do not have a bachelor's degree or higher from a country where the language of instruction is recognized as English)
- Copy of permanent resident card (if applicable)
Students applying to a certificate program (Supply Chain Management Essentials) should submit the following:
- Kelley Direct application – Part I
- Kelley Direct application – Part II (including application fee)
- Official academic records from all post-secondary institutions attended
- English translation (literal) of official academic records if not in English
- TOEFL score report (if native language is not English AND if you do not have a bachelor's degree or higher from a country where the language of instruction is recognized as English)
- Copy of permanent resident card (if applicable)
